TO PROVIDE IN-HOME AND COMMUNITY-BASED SERVICES AND SUPPORT SERVICES TO PERSONS WITH DIABILITIES AND THEIR FAMILIES. HANDZ-ON IS IN THE BUSINESS OF TAKING PEOPLE WITH PSYCHIATRIC DISABILIITIES OUT OF NURSING HOMES AND PLACING THEM BACK IN THE COMMUNITY WITH THE SUPPORT AND RESOURCES NEEDED TO MAKE THEM FUNCTIONING MEMBERS OF SOCIETY.
