1.Chosen monitors child welfare trends and addresses the increasing need for services through state-level partnerships. Across partnered states, there's a clear gap in support for foster, adoptive, kinship, and reunified parents raising children with trauma. Using telehealth, we expand access to equip families with tools they need, increasing permanency for children in the welfare system.
2.Our enhanced service model addresses the changing needs of those we serve by now supporting youth aged 11–18 in the child welfare system at risk of placement disruption. Youth Partners provides trauma-responsive, in-home support to high-acuity youth, while Parent Coaching equips caregivers with tools to stabilize placements and build healthy attachments, reducing loneliness and promoting healing.
