Vision

Through free outreach programs, community health services, and affordable access to medical care, DMHC works to reduce financial and social barriers while addressing the social determinants that influence long-term health and well-being.

Goals

1. 1.DMHC will increase access to affordable primary care for uninsured and underserved individuals by growing its patient base and Direct Care Plan enrollment while maintaining flexible non-membership options. Success will be measured by patient volume, service utilization, and improved access to preventive and routine care.
2. 2.DMHC will improve financial stability by increasing recurring revenue through Direct Care Plan growth and diversifying funding sources. Key metrics include membership growth, retention rates, and the percentage of operating costs covered by program revenue, reducing reliance on episodic fundraising.
3. 3.DMHC will expand community outreach efforts to connect underserved populations with care and resources. This includes partnerships, health education, and outreach events. Success will be measured by increased community engagement, new patient referrals, and strengthened relationships with local organizations.
